Dear Ms. Kafka
On 18July79 the individual Service Review Board met
in closed session to review your application for
discharge from the Air Force as a former member of the
Women's Airforce Service Pilots.
Attached please find a DD Form 214 (Report of Separation
from Active Duty) and a DD Form 256 AF (Honorable Discharge
Certificate). These forms are the official documents of
your discharge from the Air Force.
The IS-1 Fact Sheet attached gives helpful information
concerning your entitlement to Veterans benefits. A
copy of your DD Form 214 has been forwarded to the
Veterans Administration for their use. To apply for
Veterans benefits it will be necessary for you to contact
your nearest Veterans Administration Regional Office.
A copy of your application, supporting evidence, and
DD Form 214 will be permanently filed at the National
Personnel Records Center in St. Louis, Missouri.
HQ AFMPC/MPCAKE will retain a copy of your entire case
file and DD Form 214 for up to 3 years.
