HIDALGO COUNTY, TEXAS
ORIGINAL PETITION FOR
TERMINATION AND ADOPTION
OF CHILD
This suit is brought by
JOHNNY W. TISDALE, age 34, and
CAROL A. TISDALE, age 39,
Petitioners who are husband and
wife and who reside at 3421
Hackberry, McAllen, Hidalgo Coun-
ty, Texas.
Petitioners are not related to the
child the subject of this suit.
. 1
No court has continuing jurisdic-
tion of this suit or of this child the
subject of this suit.
n
The following child is the
subject of this suit:
NAME: BONNIE ELIZABETH
MARTIN; SEX: FEMALE; BIRTH-
PLACE: McAUen, Texas; BIRTH-
DATE: July 24, 1984; PRESENT
RESIDENCE: 3421 Hackberry,
McAllen, Texas, Hidalgo County,
Texas.
III
The mother of the child is
VIVIAN LORRAINE MARTIN, whko
is 20 years of age and whose
address is
No service is necessary at this
time.
The child is not legitimate as to
any man. An affidavit of status,
executed by the mother, is filed
herein.
The man alleged in such
affidavit to be the father or
probable father is PABLO VERA,
who is approximately 34 years of
age and whose address is unknown.
Petitioners have no further material
information as to the identity of the
biological father.
Citation of the biological father
by publication or other substan-
tiated service is necessary for the
reasons set forth in the Affidavit
filed with this petition.
There are no court-ordered
conservatorships, court-ordered
guardianships, or other court-
ordered relationships affecting the
child the subject of this suit.
IV
PABLO VERA named above, is
a non resident of Texas. The child
subject of this suit was conceived in
Texas, and such nonresident is the
father or probably father of the
child.
V
No property is owned or
possessed by the child subject of
this suit.
VI
Termination of the parent-child
relationship between VIVIAN LOR-
RAINE MARTIN and the child is in
the best interest of the child, and
such termination is requested. As
further grounds for termination,
Petitioners allege that before this
case is heard this parent will have
executed an unrevoked or irrevoca-
ble affidavit of relinquishment of
parental rights as provided by
section 15.03 of the Texas Family
Code. This affidavit of relinquish-
ment is attached to this petition.

between the child and the alleged

decree are requested.
If any parent-child relationship
does not exist, has ever existed or
could ever exist between the father
or probable father and the child, it
is in the best interest of the child
that such relationship be termina-
ted.
As grounds, Petitioners further
allege that the father or probable
father has voluntarily, and with
knowledge of thepregnancy,
abandoned the child’s : mother
beginning at a time during her
pregnancy with the child and
continuing through the birth, failed
to provide adequate support or
medical care for the mother during
the period of abandonment before
the birth of the child, and remained
apart from the child or failed to
support the child since birth.
vn
Petitioners request the Court to
appoint a guardian at litem to
represent the interests of the child.
Vin
It is in the best interest of the
child the subject of this suit to be
adopted by Petitioners, and
adoption of such child is sought.
IX
The child will not have lived in
the home of Petitioners for six
months when the case is heard.
Waiver of the requirement of six
months’ residence with Petitioners
would be in the best interest of the
child, due to the child’s tender age.
X
Petitioners request the Court
forthwith to appoint an investigator
to make and prepare a social study
into the circumstances and condi-
tion of the child and of the home of
the Petitioners and to set date for
its filing.
XI
Petitioners request that the
name of the child the subject of this
suit BONNIE ELIZABETH MAR-
TIN, who was born July 24, 1984,
be changed to BONNIE ELIZA-
BETH TISDALE.
PRAYER
Petitioners pray that citation
and notice issue as required by law.
Petitioners pray that the Court
determine what parent-child rela-
tionships exist between the child
and any other persons, that same
be terminated, that Petitioners be
granted a decree of adoption of
such child, and that any other relief
above requested be given.
Petitioners pray for general
relief.
