Rights
While you
are a patient at
Knapp Medical
Center you have
the right:
4 To receive
information
about your patient rights and responsibilities.
4 To be treated with respect and dignity. \
4 To receive medical care and services without regard to race, nation-
ality, age, gender, disability, or religion.
4 To be told about your medical condition, patient care plan and to
consent to (yes) or deny (no) treatment after receiving an explana-
tion from your doctor (an independent professional practitioner)
regarding any risks, benefits, or medical consequences of your
decision.
4 To know the identity of anyone who examines or treats you.
4 To expect privacy during medical exams, discussions, and
treatment.
4 To have your medical information confidentially maintained.
4 To look at your medical records and have them explained to you.
♦ To be told if any experimental treatments or operations are appro-
priate considerations for your care plan. You have the right to
agree OR to refuse to take part in research.
4 To request and obtain information about the existence of business
or professional relationships among health care providers that
may influence your treatment or care.
♦ To leave the hospital against medical advice, unless you have
certain diseases, or are in legal custody.
4 To know about hospital rules that affect you and your treatment
and to know about charges and payment methods.
♦ To know about hospital resources such as patient representatives or
ethics committees that can help you resolve problems and
questions about your hospital stay and care.
4 To have access to people outside the hospital by means of visitors
(your visitors must follow the hospital rules regarding visiting hours
and the no smoking policy), mail and telephone, unless medical
care reasons or legal restrictions limit or prevent such.
♦ To choose the people you wish to communicate with in regards to
visitors, hospital staff, physicians, and other health care referrals.
4 To expect that the hospital will give you necessary health care
services to the best of its ability. Treatment, referral, or transfer
may be recommended. If a transfer is recommended or requested,
you will be informed of risks, benefits, and alternatives. You will
not be transferred until the other hospital or health care facility
agrees to accept you.
4 To be put in contact with protective services. These agencies
determine the need for protective intervention when adults or
children are unable to care for themselves, have been neglected,
abused and/or exploited.
4 To be told of realistic care alternatives when hospital care is no
longer appropriate.
♦ To have an Advance Directive such as a Directive to Physicians or
Durable Power of Attorney for Health Care. By having an
advance directive, you can tell your doctor and family about the
medical care you would like to receive and whether you want
another person to be able to accept or refuse treatment for you,
just in case you are no longer able to make such decisions your-
self. If you have questions OR wish to execute an advance
directive, please ask your doctor or pick up the forms at the Infor-
mation Desk located in the front lobby of KMC.
Patient
Responsibilities
These are your responsibilities to aid in your healing and
recovery:
• To give the doctors and hospital staff accurate and complete informa-
tion about present complaints, past illnesses, hospitalizations, and
medicines you are now taking. This information should also include
any spiritual, religious, or cultural values as well as lifestyle choices
which affect your health care treatments.
♦ To understand your illness and treatments.
* To ask questions when you do not understand the information or
instructions.
♦ To follow the treatment plan recommended by the doctor primarily
responsible for your care.
* To immediately report any unexpected changes in your condition to
your doctor.
♦ To tell your doctor or other hospital staff members if you cannot or will
not be able to follow the treatment plan.
* To ensure the hospital has a copy of your advance directive if you
have completed such a document.
♦ To recognize and appreciate the impact your lifestyle has on your
personal health.
* To provide information about your insurance and work with the
hospital to arrange payment if needed.
4 To follow rules and regulations about patient and visitor conduct.
• To be polite and considerate of the rights of other patients, visitors,
staff, doctors, and volunteers.
4 To not bring money, credit cards, jewelry or other valuables. The
hospital cannot be responsible for valuables left in a patient's room.

The KMC Family Clinic provides Primary Care for all patients. The Clinic
will be staffed by a family nurse practitioner and a women's health nurse
practitioner. Dionisio B. Calvo III M.D., an independent professional
practitioner, is the medical director.

Hospice Care Services Coming Soon To KMC
Knapp Medical Center will soon offer Hospice Care Services. Hospice Care Services will
provide a special kind of caring for people with terminal illnesses and their families. The program
will focus on providing a comfortable and supportive environment by a team of professionals—
doctors, nurses, chaplains, social workers, therapists, dietitians, volunteers, and others. Referrals to
KMC's Hospice Care Services may be requested by a patient, physician (an independent professional
practitioner), family member, discharge planner, social worker, friend, neighbor, nurse or clergy.
