Tentatively Okays Budget
Hospital Board
Sets Tax Rate

Establishing the tax rate and
tentative approval of a budget for
1985-1986 were among the actions
taken by the Hospital Board at
their regular meeting Thursday
night.
Deep into the meeting, Ad-
ministrator Thalia Munoz noted
that "valuations went up a little
bit." She said that raising the tax
rate three percent would mean a
rate of .0582 per $100 valuation.
Munoz said that "the only
difference eight percent would
make is $19,233." Vice-President
Ross LaGrange then moved that
the three percent increase to .0582
be accepted.
The board then discussed the
proposed budget in some detail.
Board member Romeo Alaniz
expressed his differences of opi-
nion, most notably that $108,000
was charged as a depreciation
expense. He said, "How can you be
deducting off a budget?"
Business Manager Inez Hinojosa
said, "It's an expense to the
operating account." Attorney
David Guerrero said, "When you
have to replace the equipment, it
will be paid for from the operating
fund." Later, Munoz called the
depreciation expense entry "stan-
dard procedure", saying the of-
fsetting entry is in the Equipment
and Plant Fund
When Hinojosa said that a five
percent increase is being projected
in the nursing department, Alaniz
said the budget actually showed a
nine percent rise. When this and
several other errors were noted,
Munoz and Hinojosa said the
budget would be corrected.
Among the other errors Alaniz
wondered about was that $69,000 is
budgeted for dietary supplies, after
$119,000 was slated to be spent in
1984-1985. Something else ques-
tioned by Alaniz was the 32 percent
increase budgeted for deliveries.
He said, "We're supposed to have a
down trend due to the new private
clinic."
After this discussion, Alaniz
moved, seconded by LaGrange.

In other business, Munoz noted
that "the second ambulance isn't
here yet," but that Southwest
Vehicles had promised delivery by
Aug 21 The administrator said she
was pleased that Mike Smith, who
taught the recent Emergency Med-
ical Technician classes, will come
from Laredo to work with the
hospital's new ambulance service.
Munoz said of Smith, "He's a
paramedic with a lot of experi
ence." She said that four LVN's
recently took the EMT class and
passed, and are now willing to
| work at least part-time. As for
other help, she said that Ceballos
employees have been asked if they
can work on their days off
Alaniz said he thought a number
of persons in the Rio Grande City
area will be willing, even eager, to
volunteer their services He sug-
gested that hospital security
guards could be trained for am-
bulance work LaGrange suggested
utilizing Rio volunteer firemen
) part-time. Munoz said that at the
current time, "We have two people
full time."
About insuring the ambulances,
Munoz said. "There's very few
companies that will insure am-
bulances. It has to be the whole
thing, building and contents. She
said that a proposal had been sent
by Garza Insurance, and "they
quoted a price of $4428 16 to insure

The board then okayed the Garza
Insurance bid for collision and
liability insurance. Liability insur-
ance will be in the range of
$100,000, $300,000, and $100,000,
while personal injury will be $2500.
About the proposed underground
gasoline tank, attorney Roger Reed
said, "There's nothing in the
statutes to require you to go out for
bids. We're a separate political
subdivision." Munoz noted that
Gold Star's proposal was for
$18,500, while Arturo Longoria
quoted $17,500.
Munoz and everyone else
expressed surprise that I,&L
Company quoted a price of $7492.
Munoz said, "I cannot believe
this," and Alaniz warned, "They
may make up the difference in
gasoline later on."
Despite these concerns, Alaniz
moved that the L&L bid be
accepted, pending the obtaining of
a firm contract in writing.
Munoz said, "We do have the
computer in We've had two groups
help with in-service" in areas like
accounts receivable. About equip-
ping the ambulances with radios,
she said that has been "put off,"
and added that Mike Smith "has
much experience with radios, he
should have influence in making a
decision "
The dispute with the Water
District about $45,000 in
overcharges from October 1981 to
1984 appears to be over according
to Munoz She and the board noted
a letter from Manager Efrain
Duran that called for the
overcharges to be returned in
gallons later Munoz said later,
"We'll get free water for a while,
water that's already been paid
for."
Discussion arose about what had
caused the erroneous billing
Munoz expressed her opinion that
"the discrepancy i^ probably with
the meter." LaGrange wonders il
there were leaks in underground
pipelines. Hinojosa said the meter
doesn't move until the big tank
starts operating.
Reed said, "Is the ground per-
meable enough to absorb two to
three million gallons?" Alaniz
suggested, "Make them change the
meter." In any case, Munoz noted
that current bills are in line with
meter readings. The board then
voted to accept the Water District's
plan to settle the problem.
Munoz said the hospital went
through a meticulous Medicare
survey July 23 and 24. She said that
some problems turned up, among
them the need to update medical
bylaws, record-keeping, and en-
forcement of medical staff privi-
leges Munoz said, "Another area
they hit really hard was
pharmacy," but added "It's
nothing we can't correct."
She s aid that the surveyors' main
concern is possible abuse of con-
trolled substances. For example, if
five milligrams of a 10-milligram
dose are used on a patient, the
other five milligrams must be
thrown away. Munoz said,
"Whatever we can, we're ordering
in smaller quantities."
Medical Chief of Staff Sergio
Rodriguez said, "We took some hits
(from the surveyors) concerning
overall recordkeeping and what's
actually there. Another problem
was enforcement of medical staff
privileges. The criticisms are
valid, we got our work cut out.
Everything they criticized had
been discussed in previous medical
staff meetings."
Among the equipment sought by
the hospital are radios, stretchers,
rescue tools and resusitators for
the ambulances a coulter counter
and coagulation instrument for the
laboratory: a cardiac monitor and
defibrillator for the nursing de-
partment; and also a thumper
cardiopulmonary life support
equipment.
